Not every participant needs the same report. A person seeking a few more support hours needs something different from a person whose living arrangement is under review, and different again from someone who needs a complex home modification signed off.
Our Logan Occupational Therapists start by working out what decision the assessment is actually feeding, a plan review, a housing change, an equipment request, a change in support model, and then complete the assessment that will genuinely evidence it. Getting that wrong is expensive: it burns plan funding and delays the outcome by months.
We assess in the participant's own home and community, so what we recommend reflects how they genuinely manage rather than how they present in an unfamiliar room.

Housing, SIL and SDA Assessments
Housing decisions are among the highest-stakes decisions in a participant's plan, and among the hardest to reverse. They deserve a properly reasoned assessment.
Where a participant's support model or living arrangement is under consideration, our Logan Occupational Therapists complete the functional and housing assessments required and prepare reports that set out the evidence clearly, what the participant can do, what support they need and when, what the current environment does and does not provide, and why the recommended arrangement follows from that.
We complete Supported Independent Living (SIL) assessments and reports, Specialist Disability Accommodation (SDA) assessments and reports, housing and living arrangement reports, and home environment and access assessments. We are equally comfortable saying when a recommendation is not supported by the evidence, which is ultimately more useful to everyone than a report that does not hold up.

Equipment & Environment
Assistive Technology, Seating and Home Modifications
Equipment and environment do work that no amount of therapy can. Our Occupational Therapists complete assistive technology assessment, trial and prescription, from daily living aids through to wheelchairs, complex seating, pressure care and transfer equipment, and prepare the reports and quotes required for funding requests.
For home modifications we assess the participant against the property and specify what is required, from grab rails, step ramps and handrails through to level-access showers, bathroom reconfiguration, widened doorways and access ramps. We review the outcome once equipment arrives or the work is complete, to confirm it does what it was meant to do.
Where a participant also sees a physiotherapist, our clinicians assess jointly on seating, positioning, transfers and manual handling, which produces better equipment decisions than either discipline reaches alone.

Building Everyday Independence
Alongside assessment and reporting, we deliver capacity building therapy, the work that reduces the support a participant needs rather than simply funding more of it. That includes personal care and daily living skills, meal preparation and kitchen safety, household management, medication and health routines, money handling and appointments, routine and organisation, community access and transport, sensory and regulation strategies, safety awareness, fatigue management, and training for families and support workers.
