Most of the risks that force an older person out of their home are practical ones. A step at the front door. A shower without a rail. A bed that is too low to stand up from. A kitchen that has become unsafe. An occupational therapist's job is to find those risks before they become a fall, a hospital admission or a permanent move — and to fix them.
Our Gold Coast Occupational Therapists assess people in their actual home, not a clinic room. We watch how someone genuinely gets out of bed, into the shower, up the hallway and out to the letterbox, and we build recommendations around what we see. AT-HM Scheme Assessments
Assessment, prescription and clinical reporting for assistive technology and home modifications funded through the AT-HM scheme — from grab rails and ramps to hospital beds and stair lifts.
Falls Prevention & Home Safety
Whole-person home safety assessments that look at the person, the task and the environment, producing a plan that addresses equipment, modifications, technique and the right amount of support.
Reablement After Hospital

Occupational Therapy Under Support at Home
Occupational therapy sits within the clinical care category of the Support at Home program, which replaced Home Care Packages from 1 November 2025.
That makes OT one of the most accessible and most useful services in an older person's Support at Home budget. Our Gold Coast Occupational Therapists provide a full range of clinical and home safety services, and we also work with residential aged care providers across the Gold Coast and accept private referrals from people who would rather not wait for funding to be approved.

Funded Entitlement
Assistive Technology and Home Modifications (AT-HM) Scheme
The AT-HM scheme provides funding for equipment and home modifications that is separate from, and upfront of, the rest of a person's Support at Home budget. It is one of the most under-used entitlements in aged care, largely because people do not know it exists. Funding is provided across three tiers within a 12-month access period:
Low tier
up to $500
Medium tier
up to $2,000
High tier
up to $15,000
More complex equipment and any home modifications generally require assessment and prescription by a health professional — usually an occupational therapist. That is where we come in.
Our Occupational Therapists complete the assessment, document the clinical justification, specify what is required, and prepare the report needed to support the funding request — then review the outcome once the work is complete to make sure it actually works for the person.

Falls Prevention & Home Safety
A fall is rarely caused by one thing. It is usually the combination of reduced strength, a medication side effect, poor lighting, an unfamiliar rug and a task the person shouldn't have been doing alone.
Our home safety and falls prevention assessments look at the whole picture — the person, the task and the environment — and produce a plan that addresses all three. That may include equipment, modifications, changes to how a task is done, referral to physiotherapy for strength and balance work, education for family, or a change to the support schedule.
After Hospital: Reablement
Coming home after a hospital admission is the point at which many older people lose independence permanently. Early occupational therapy makes a measurable difference.
We provide short-term, goal-focused reablement following hospital discharge, illness, surgery or a fall — rebuilding confidence and function in showering, dressing, meal preparation, mobility and community access, and putting the right equipment in place before problems set in. Referrals from hospital discharge planners, transition care teams and aged care providers are welcome.
Living Well with Dementia & Cognitive Change
Cognitive change affects function long before it affects memory alone. Our Occupational Therapists assess how dementia, mild cognitive impairment, stroke or other conditions are affecting safety, routine, decision-making and daily activity, and provide:
- Environmental modifications that reduce confusion and risk
- Simplified routines and task set-ups that support independence
- Kitchen, medication and wandering safety strategies
- Meaningful activity and engagement planning
- Practical education and strategies for family and carers
- Recommendations for the level and type of support required
