A Wound That Isn't Healing Is Telling You Something
Persistent wounds are rarely a dressing problem. They are usually a circulation problem, a pressure problem, a nutrition problem, a diabetes problem or an infection problem wearing a dressing. Change the dressing every few days without addressing the cause and the wound will sit there for months.
Our Melbourne wound care nurses assess both. We examine the wound properly, size, depth, tissue type, exudate, wound edge, surrounding skin, and then look at why it is not progressing. Circulation, pressure loading, glycaemic control, nutrition, oedema, footwear, mobility, medication. The dressing plan follows from that assessment rather than from habit.
We also measure and photograph at each review, so progress is visible rather than guessed at, and so your GP can see what is actually happening between appointments. Where a wound is not responding as it should, we escalate rather than continue.

Getting Started
Make a referral
Individuals, families and carers, GPs and specialists, hospital discharge planners, aged care providers and care partners, support coordinators and plan managers can all refer. Tell us the wound type, how long it has been present and what has been tried.
We call you for an initial discussion
One of our nursing team calls you to gather the clinical detail, the wound type and location, how long it has been present, what dressings have been tried, whether vascular assessment has been done, and your GP's details. We confirm funding and explain what the first visit will involve.
We match the nurse
Your referral is allocated to a Melbourne Registered Nurse with wound management experience relevant to the wound type.
Full wound assessment
The nurse assesses the wound and the reasons it is not healing, reviews current management, photographs and measures, and develops a wound care plan with you and your GP.
Ongoing dressing and review
Visits are scheduled around the dressing regimen and reviewed as the wound changes. Family and support workers are shown what they need to know.
Progress reporting
Wound progress is documented and reported to your GP, specialist or care coordinator, with escalation where the wound is not responding.

Specialist Therapy
Graduated Compression Therapy
Compression is the single most effective treatment for venous leg ulcers, and one of the most commonly under-used. Many long-standing leg ulcers heal once adequate compression is applied consistently.
It also has to be applied safely. Compression is not appropriate where arterial supply is compromised, so our nurses confirm that vascular assessment has been completed and work under medical direction before commencing compression therapy. Where arterial assessment has not been done, we arrange it with your GP rather than proceeding on assumption.
Once compression is underway we manage bandage or garment application, monitor skin integrity and tolerance, adjust as oedema reduces, and educate the person and family on wear, care and warning signs, because the therapy only works if it continues between visits.
Collaborative Care
Diabetic Foot Wounds
A diabetic foot wound is a serious event. Neuropathy means it may not hurt, poor circulation means it may not heal, and infection can progress quickly with few obvious symptoms.
Our nurses manage diabetic foot wounds in collaboration with our Podiatry team, combining wound management with offloading, footwear review, high-risk foot screening and glycaemic control support. We also educate on daily foot checking, because the next wound is usually preventable.
Pressure Injury Prevention and Management
Pressure injuries develop fast in people with reduced mobility, and they are far easier to prevent than to heal. Our nurses assess pressure risk, manage existing injuries, review repositioning and equipment, check heels and other high-risk areas, and work with our occupational therapists on pressure-relieving mattresses, cushions and seating where equipment is part of the answer.

Education for Families and Support Workers
Most wound care happens when we are not there. We show families and support workers what a healthy wound looks like, what deterioration looks like, how to protect the dressing, what to do if it comes off, and when to call, so nobody is left guessing between visits.
