Complex Wound Care
Assessment and management of chronic and complex wounds, including leg ulcers, pressure injuries, diabetic foot wounds and surgical wounds that are not healing as expected.
Continence Assessment
Comprehensive continence assessment leading to a practical plan, the right products, the right strategies, and referral where the underlying cause can be treated.
Medication Management
Medication administration, injections, dose administration aid management, and monitoring for people managing complex or high-risk medication regimens.
Ongoing Clinical Care
Catheter and stoma care, enteral feeding support, diabetes management, health monitoring, care planning and clinical review in the home.

Wounds that are not healing are rarely a dressing problem. They are usually a circulation problem, a pressure problem, a nutrition problem, a diabetes problem or an infection problem wearing a dressing.
Our nurses assess the wound and the reasons behind it, then manage both. That means proper wound assessment and measurement, appropriate dressing selection and review, pressure offloading, escalation to the GP or specialist when something is not responding, and photographic documentation so change over time is visible rather than guessed at.

Continence
Continence Assessment
Continence problems are common, under-reported and frequently treatable, and they are one of the strongest predictors of a move into residential care. They are also, too often, managed by simply buying more pads.
Our nurses complete comprehensive continence assessments that identify what is actually happening and why. That includes bladder and bowel history, fluid and dietary review, medication review for continence-related side effects, mobility and access to the toilet, cognition and communication, skin integrity, and the suitability of current products.
We complete continence assessments for older Australians under aged care funding, and continence assessments and product reports for NDIS participants where required.
The outcome
The outcome is a written continence plan: the strategies to try, the products that genuinely suit the person's pattern and body, environmental or equipment changes that improve access, and referral onward where the underlying cause can be treated medically or through pelvic health physiotherapy.
Funding
Funding and How Nursing Is Paid For
Nursing sits within the clinical care category of the Support at Home program, which replaced Home Care Packages from 1 November 2025. Clinical care is fully funded by the government regardless of income or assets, there is no means-tested contribution for nursing care.
For older Australians, that makes in-home nursing one of the most accessible services in a Support at Home budget, and one of the most cost-effective ways to avoid preventable hospital admissions. We work alongside Support at Home providers and care partners, and can provide clinical reports, care plan input and progress updates directly to them.
We also accept NDIS participants where nursing supports are funded in the plan, residential aged care referrals, and private referrals from people who would rather not wait for funding approval.

How to Get Started
Make a referral
Older Australians, families and carers, aged care providers, care partners, GPs and specialists, hospital discharge planners and support providers can all refer.
We match the nurse
Your referral is allocated to a Sydney nurse with experience relevant to the clinical need, wound management, continence, medication or complex care.
Initial visit and assessment
The nurse assesses in the home, reviews current management and medications, and develops a clear plan in consultation with you and your doctor.
Ongoing care
Visits are scheduled around the clinical need and reviewed as the situation changes. Family and support workers are shown what they need to know.
Reporting and review
Progress is documented and reported to your GP, care partner or provider, with escalation where something is not responding as expected.
