Families rarely change their minds about home care because they stop wanting it. They change their minds at 2am, when something happens that nobody prepared them for and they do not know what to do.
Good palliative nursing at home is largely about removing those moments in advance. Making sure medications are on hand before they are needed. Making sure the family knows what breathing changes mean, what to expect in the last days, and exactly who to call and when. Making sure the bed, the equipment and the room are set up before they become urgent.

Getting Started
Make a referral
Patients, families and carers, GPs and specialists, palliative care services, hospital discharge planners, aged care providers and care partners can all refer. Say that it is a palliative referral so we prioritise it.
We call you for an initial discussion
One of our nursing team calls you promptly to understand the situation, where the person is up to, which palliative care service and GP are involved, what symptoms are being managed, what equipment is in place, and what the family needs most right now. We confirm funding and arrange the first visit quickly.
We match the nurse
Your referral is allocated to a Melbourne Registered Nurse experienced in palliative and end-of-life care.
First visit and planning
The nurse assesses symptoms and comfort, reviews medications and equipment, checks the home set-up, and talks with the family about what to expect and what they need.
Ongoing nursing care
Visits are scheduled around the person's needs and adjusted as things change, with close contact maintained with your palliative care service and GP.
Support through and after
We support the family through the final stage and make sure they know who to call and what happens next.

What Our Palliative Nursing Includes
Supporting the Family and Carers
Carers of someone dying at home are doing skilled work, usually without training, usually exhausted, and usually frightened of getting it wrong.
We take that seriously. Our nurses show families the practical things, how to reposition someone comfortably, how to manage mouth care, what to do if a dressing comes off, how to recognise pain in someone who can no longer report it. We answer the questions people are reluctant to ask, including about what the last days and hours usually look like, because not knowing is nearly always worse than knowing.
We also watch the carer. Carer collapse is one of the most common reasons a home death does not happen, and it is often preventable with respite, extra support hours or a frank conversation early enough to arrange them.
Coordinated Care
Working With Your Palliative Care Service
We complement specialist palliative care rather than replacing it. Your specialist palliative care service and GP set the symptom management plan and provide medical oversight, including after-hours clinical advice. Our nurses deliver nursing care within that plan, feed observations back, and escalate promptly when something changes.
Our nursing visits are scheduled during our operating hours, Monday to Saturday, 9:00am, 5:00pm. Your palliative care service will provide the after-hours contact for urgent clinical advice, and we make sure that number is written down and visible in the home from the first visit.
If you do not yet have a specialist palliative care service involved, we can talk you through how to get a referral through your GP or treating specialist.
The Support at Home End-of-Life Pathway
Support at Home includes a dedicated End-of-Life Pathway for people who have been assessed by a medical practitioner or nurse practitioner as having three months or less to live. It provides faster access to services through My Aged Care, with funding of $25,686.25 available over a 12-week period, extendable by a further four weeks where needed.
Clinical care, including nursing, is fully funded with no participant contribution. Independence and everyday living services within the pathway are means tested in the usual way. Assistive technology can also be funded where it is required.
Accessing the pathway requires a completed End-of-Life Pathway form, submitted through the My Aged Care referral tool or during a high-priority aged care assessment. If this is relevant to your situation, speak to your GP or treating team, and our intake team can help you understand how nursing fits within it.

Our clinic is based in Roxburgh Park, and we provide palliative and end-of-life nursing across Melbourne's northern suburbs, Craigieburn, Greenvale, Broadmeadows, Meadow Heights, Mickleham, Kalkallo, Epping, Mill Park, Glenroy, Coburg and the surrounding corridor.
Responsiveness matters more in palliative care than in any other part of our work. Being based in Melbourne's north means we can usually get to families in the northern suburbs quickly when something changes, rather than the following day. Many families in the corridor also want cultural and religious practices respected at the end of life, and we ask about that early rather than assuming.
