Make a referral
Individuals, families and carers, GPs and pharmacists, hospital discharge planners, aged care providers and care partners, support coordinators and plan managers can all refer.
We call you for an initial discussion
One of our nursing team calls you to gather the detail, the current medication list, which pharmacy supplies the pack, any recent hospital stay or script changes, who is helping at the moment, and your GP's details. We confirm funding and explain what the first visit will involve.
We match the nurse
Your referral is allocated to a Melbourne Registered Nurse with experience relevant to the medication regimen.
Medication assessment
The nurse establishes what is actually being taken, reconciles it against current prescriptions and any discharge summary, and identifies risks, duplications, omissions, interactions or storage problems.
A workable plan
We agree what the nurse will administer, what can be prompted, how the pack will be managed, and what family or support workers need to know, then document it clearly for the household.
Ongoing review and reporting
Visits continue at the frequency the regimen requires, with reporting to your GP and escalation whenever something needs medical review.

What We Provide
After a Hospital Stay
Discharge is the highest-risk point in anyone's medication history. Doses change, new medicines start, old ones stop, and the webster pack at home still reflects the arrangement from a fortnight ago.
Our nurses reconcile the discharge summary against what is physically in the house, identify duplications and omissions, arrange a corrected pack with the pharmacy, and explain the changes to the person and their family. Where a discharge medication looks wrong or a critical medicine has been stopped without explanation, we go back to the GP rather than assume.
Specialist Support
Supporting People With Cognitive Impairment
For people living with dementia or other cognitive change, the problem is rarely willingness, it is memory, sequencing and recognition. Medications get taken twice, or not at all, or the person is certain they have already had them.
We work on the practical set-up: simplified regimens discussed with the GP, dose administration aids, visual and routine prompts, secure storage where doubling up is a risk, timing that fits the person's day, and clear guidance for family and support workers on what to do when a dose is refused or missed. Where the level of risk means administration by a nurse is warranted rather than prompting, we say so.
Working Together
Working With Support Workers
Many people have support workers present daily and a nurse present weekly. Getting the boundary right matters, support workers can generally assist with medication within their scope and their provider's policy, but there are limits, and those limits exist for good reason.
Our nurses provide education and competency support for support workers, clear written medication guidance for the household, and take on the tasks that require a registered nurse. Where a provider's staff are being asked to do something outside their scope, we will say so plainly.

Melbourne's north is one of the most culturally and linguistically diverse parts of the city, and medication understanding is exactly where that matters.
