In-home continence assessments and individualised management plans across Melbourne and Roxburgh Park, preserving dignity and supporting people to remain at home longer.
Continence problems are common, under-reported and frequently treatable. They are also one of the strongest predictors of a move into residential care, and too often they are managed by simply buying more pads.

inOne Healthcare
Continence is the health problem people are least likely to raise. It gets managed privately for years, worked around, hidden from family, and eventually mentioned only when it has already narrowed someone's life, they have stopped going out, stopped travelling, stopped seeing people, or started sleeping in a different room.
By that point the person has usually concluded it is just part of getting older, or part of their condition, and cannot be helped. That is frequently untrue. Many continence problems improve substantially with the right assessment, and almost all of them can be managed far better than they currently are.
Our Melbourne continence nurses assess without fuss and without embarrassment. It is a clinical problem with clinical causes, and we treat it that way.

What We Do
A full clinical assessment of bladder and bowel function, contributing factors and current management, completed in the person's own home.
A written plan covering strategies, timing, fluids, toileting routines and environmental changes, not just a product recommendation.
Identifying products that genuinely suit the person's pattern, body and skin, so funding is not spent on products that do not work.
Continence assessment reports for NDIS participants and clinical documentation for aged care providers and care partners.

Simple Process
Individuals, families and carers, GPs, aged care providers and care partners, support coordinators and plan managers, and support providers can all refer. You can refer for yourself, no GP letter is required.
One of our nursing team calls you for a discreet conversation about what is happening, the pattern, how long it has been going on, what products are being used and how it is affecting daily life. We confirm funding and explain exactly what the assessment involves, so there are no surprises.
Your referral is allocated to a Melbourne Registered Nurse experienced in continence assessment and management.
The nurse completes a full continence assessment in your own home, including the bathroom and the practical realities of getting there. It takes around an hour and is done discreetly.
You receive a written, individualised management plan covering strategies, products, environmental changes and any referrals needed.
We review how the plan is working and adjust it, because continence needs change over time and a plan that fitted a year ago often does not fit now.

Our Services
NDIS participants often need a continence assessment report to evidence funding for continence products or related supports. Our nurses complete the assessment and prepare the report, documenting the clinical picture, the functional impact, what has been trialled, and why the recommended products and supports are appropriate.
We work with self-managed, plan-managed and NDIA-managed participants, and liaise directly with support coordinators and plan managers.
For older Australians, continence is one of the most significant factors in whether someone can keep living at home, and one of the biggest sources of carer strain. Nursing sits within the clinical care category of Support at Home, which the government funds in full, so a proper continence assessment costs the person nothing.
We also work with residential aged care providers on continence assessment, management planning and staff education.

Local Presence
Our clinic is based in Roxburgh Park, and we complete continence assessments across Melbourne's northern suburbs, Craigieburn, Greenvale, Broadmeadows, Meadow Heights, Mickleham, Kalkallo, Epping, Thomastown, Glenroy, Coburg and the surrounding corridor.
Continence is a subject people find hard to raise, and harder again where it carries cultural stigma or where the conversation has to happen through a family member. Melbourne's north is highly multicultural, and we work with interpreters where they are needed, arrange a nurse the person will be comfortable with where we can, and keep the assessment private and matter-of-fact. In-clinic appointments at Roxburgh Park are available for anyone who would prefer not to be assessed at home.
Service Area
Our continence nurses provide home visits across all Melbourne suburbs, with our clinic based in Roxburgh Park in Melbourne's northern suburbs.
