In-Home Occupational Therapy for Older Ipswich Residents
inOne Healthcare's Community Occupational Therapists visit people at home throughout Ipswich, Springfield, Ripley and the surrounding towns. We work with older Australians receiving aged care funding, their families and carers, and their aged care providers with a strong focus on falls prevention, home safety, equipment prescription and home modifications funded through the Assistive Technology and Home Modifications (AT-HM) scheme.

Falls are rarely caused by one thing. They are usually the combination of reduced strength, a medication side effect, poor lighting, a loose mat and a task the person should not have been attempting alone. Our assessments consider the person, the task and the environment together.
Reablement After Hospital

Assistive Technology and Home Modifications (AT-HM) Scheme
AT-HM funding sits separate from, and upfront of, the rest of a person's Support at Home budget. It exists specifically to pay for the equipment and home modifications that keep people safe at home — and it is regularly left unused simply because people do not know it is there.
Funding is provided across three tiers within a 12-month access period:
Low tier
up to $500
Medium tier
up to $2,000
High tier
up to $15,000
Complex equipment and any home modification generally require assessment and prescription by a health professional — typically an occupational therapist.
We complete the assessment, document the clinical justification, specify exactly what is required, prepare the report to support the funding request, and review the outcome once the equipment arrives or the work is finished.

Falls Prevention & Home Safety
Falls are rarely caused by one thing. They are usually the combination of reduced strength, a medication side effect, poor lighting, a loose mat and a task the person should not have been attempting alone.
Our home safety and falls prevention assessments consider the person, the task and the environment together, and produce a plan that addresses all three — equipment, modifications, changes to how tasks are done, referral for strength and balance work, carer education, and adjustments to the support schedule.
After Hospital: Reablement and Restorative Care
The weeks after a hospital admission are when older people most often lose independence for good. Early occupational therapy changes that trajectory.
We deliver short-term, goal-focused reablement after hospital discharge, illness, surgery or a fall — rebuilding confidence and function in showering, dressing, meal preparation, mobility and getting back out into the community, with the right equipment in place before problems become permanent. We welcome referrals from Ipswich Hospital and St Andrew's discharge planners, transition care teams and aged care providers.
Living Well with Dementia and Cognitive Change
Cognitive change affects daily function long before it becomes obvious. Our Occupational Therapists assess how dementia, mild cognitive impairment, stroke or other conditions are affecting safety, routine, decision-making and daily activity, and provide:
- Environmental modifications that reduce confusion and risk
- Simplified routines and task set-ups that support independence

NDIS Occupational Therapy in Ipswich
We also support NDIS participants throughout Ipswich. Our Occupational Therapists complete Functional Capacity Assessments, home modification and assistive technology assessments, SIL and SDA reports, and provide ongoing capacity-building therapy for adults with neurological, cognitive and functional conditions. We work with plan-managed and self-managed participants and liaise directly with support coordinators and plan managers.
