Strength and balance decline quietly. Most people do not notice until they are holding the furniture on the way to the kitchen, or have stopped going out because the kerbs and steps feel unsafe. By then the loss is significant but it is also, in most cases, reversible. Physiotherapy is the single most effective intervention for that decline, and our physiotherapists assess and treat in the home, where we can see the real stairs, the real chair height, the real bathroom and the real distance to the front gate, then build a program that targets exactly what is limiting the person.
Western Sydney is also one of the most culturally and linguistically diverse regions in the country, and many older people here live in multigenerational households. We work with interpreters where they are needed, involve the family members doing the day-to-day care, and design programs that fit how the household actually runs. Every referral is matched to a physiotherapist with relevant clinical experience, and we work collaboratively with participants, families, carers, support coordinators, GPs, specialists, support workers and other allied health clinicians.

Support at Home
Physiotherapy sits within the clinical care category of the Support at Home program, which replaced Home Care Packages from 1 November 2025. Clinical care services are fully funded by the government regardless of income or assets — there is no means-tested contribution for physiotherapy.
Our Western Sydney physiotherapists provide the services listed here, and we also work with residential aged care providers across Western Sydney. We accept private referrals from people who would rather not wait for funding approval.
Falls & Balance
Around one in three older Australians falls each year, and a fall is very often the event that ends independent living. Falls are also one of the most modifiable risks in aged care.
Effective falls prevention is not a leaflet and a handrail. It is progressive, challenging, correctly dosed balance and strength training, delivered consistently over enough weeks to change the underlying capacity — combined with gait retraining, footwear and mobility aid review, and environmental risk identification alongside our occupational therapists.
Our physiotherapists also screen for the contributors that often go unnoticed: postural blood pressure changes, vestibular symptoms, reduced walking speed, fear of falling, and the activity avoidance that quietly accelerates decline.
Strength
Loss of muscle mass and power with age is not inevitable and it is highly treatable. Resistance training works at any age, including in people in their nineties and people managing multiple chronic conditions.
Our programs are progressive and individually dosed — starting where the person is, building steadily, and continuing as a maintainable home program with family or support workers assisting where needed. We work on the specific tasks that matter to the person, whether that is managing the front steps, carrying shopping, getting off the floor, or getting to the local shops and back.
Rehabilitation
The weeks after a hospital admission are when function is most recoverable — and most easily lost for good. Early physiotherapy changes that trajectory.
We provide in-home rehabilitation after joint replacement, fracture, stroke, cardiac and respiratory admissions, prolonged illness, ICU stays or a general decline in condition. Programs are short, goal-focused and measurable. We welcome referrals from Westmead, Blacktown, Nepean, Liverpool and Campbelltown discharge planners, transition care teams and aged care providers.
Pain & Chronic Conditions
Persistent pain is one of the main reasons older people stop moving, and stopping moving makes almost everything worse. Our physiotherapists assess and treat arthritis, back and neck pain, shoulder and hip pain, and persistent pain conditions — combining manual therapy, graded activity, exercise and education so that people stay active rather than gradually withdrawing.
We also support people managing osteoporosis, respiratory conditions, cardiac conditions, diabetes and deconditioning, with exercise programs designed around their capacity and their other health needs.
NDIS
We also support NDIS participants throughout Western Sydney. Our physiotherapists provide neurological rehabilitation for conditions including stroke, acquired brain injury, multiple sclerosis, Parkinson's disease, spinal cord injury and cerebral palsy, along with:

Simple Process
Older Australians, families, aged care providers, care partners, GPs and hospital teams can all refer. No referral is required to see a physiotherapist.
Your referral is allocated to a Western Sydney physiotherapist with the relevant experience.
We assess strength, balance, mobility, pain and function in the home environment, and agree on goals. An interpreter can be arranged if needed.
Treatment sessions plus a home program designed around what the person will realistically do, with family or support workers trained to assist.
Progress is measured and reviewed, with reports to care partners and providers as needed.
