Most rehabilitation ends at the hospital door. The person goes home, the supervised program stops, and the gains made over weeks start unwinding within days, not because the person stopped trying, but because nobody translated the program into the house they actually live in.

Our Sydney Physiotherapists work on the other side of that door. We assess how a person moves through their own home, the step at the entry, the narrow hallway, the low lounge, the bathroom with nothing to hold, and build treatment around it. Nothing has to be generalised from a gym to a home, because the work happens in the home to begin with.

We support NDIS participants building movement capacity toward their own goals, and older Australians funded through aged care who intend to keep living safely where they are.

NDIS

NDIS Physiotherapy in Sydney

For NDIS participants, physiotherapy is about capacity, the physical ability to do more, with less support, more safely, for longer. Where a condition is progressive, the goal shifts to maintaining function and comfort for as long as possible, and we are explicit about which of those we are working toward.

We work with participants at home, in the community and in supported accommodation, and we build programs that a support team can deliver consistently between our visits. Physiotherapy is typically funded under Capacity Building, Improved Daily Living, with Improved Health and Wellbeing also relevant for some participants. We support self-managed, plan-managed and NDIA-managed participants and work directly with support coordinators and plan managers.

Physiotherapy for Older Australians in Sydney

Strength and balance decline quietly. Most people do not notice until they are holding the furniture on the way to the kitchen, or have stopped going out because the steps feel unsafe. By then the loss is significant, and in most cases it is reversible.

Resistance training works at any age. We regularly build measurable strength in people in their eighties and nineties, including those managing several chronic conditions. What matters is that the dose is right and the progression is real.

Support at Home funding

Physiotherapy sits within the clinical care category of the Support at Home program, which replaced Home Care Packages from 1 November 2025. Clinical care is fully funded by the government regardless of income or assets, there is no means-tested contribution.

Rehabilitation

Rehabilitation After Hospital, Surgery or a Fall

The weeks after discharge are when function is most recoverable and most easily lost for good. We provide in-home rehabilitation after joint replacement, fracture, stroke, cardiac and respiratory admissions, prolonged illness, ICU stays or general decline.

Programs are short, goal-focused and measurable. We welcome referrals from hospital discharge planners, rehabilitation teams, transition care services and aged care providers across Sydney.
