Working With Your Plan and Your Team

Physiotherapy is typically funded under Capacity Building, Improved Daily Living, with Improved Health and Wellbeing also relevant for some participants. We support self-managed, plan-managed and NDIA-managed participants.

We work as part of a team rather than in isolation. That means talking to support coordinators about what is realistic, briefing support workers so they can carry the program between visits, coordinating with occupational therapy on equipment and positioning, and liaising with GPs, specialists and rehabilitation services where a participant's medical care is ongoing.

We provide functional assessments and progress reports that set out what has changed, what has not, and what is required next, written so a planner can follow the reasoning from the participant's condition, through its functional impact, to the specific support being recommended. Where equipment, additional therapy hours or support worker assistance is needed, we say so plainly and explain why.

Participants can refer themselves. We also accept referrals from support coordinators, plan managers, families and carers, GPs and specialists, hospitals and rehabilitation teams, and support providers.

Your referral is allocated to a Sunshine Coast Physiotherapist with experience relevant to the participant's condition and goals.

Initial assessment at home

We assess movement, strength, balance, mobility and function in the participant's own environment, and agree on goals that matter to them.

Treatment sessions plus a home program designed around what the participant will realistically do, with family and support workers trained to assist.

Progress is measured against the agreed goals, with reports provided for plan reviews and the wider support team.
