An Assessment Is Only As Good As Where It Happens

Ask someone whether they can manage their bathroom and they will usually say yes. Watch them do it and you will often see something else entirely, a hand on a towel rail that was never designed to bear weight, a step over a hob that takes three attempts, a chair dragged in because standing that long is no longer possible.

That gap is why our Sydney Occupational Therapists assess in the home. Not because it is more convenient, though it is, but because it is the only way to see how a person genuinely manages. Recommendations built on observation hold up. Recommendations built on self-report often do not.

We work with two groups across Sydney: older Australians funded through aged care who intend to stay living at home, and NDIS participants working toward their own goals.

Occupational Therapy for Older Australians in Sydney

Most of what forces an older person out of their home is practical. A step at the entry. A shower without a rail. A bed too low to stand up from. A kitchen that has quietly become unsafe. Our job is to find those risks before they become a fall, an admission or a permanent move, and then fix them.

Sydney housing makes this specific. Terraces with stairs and no ground-floor bathroom, walk-up apartments without lifts, older units with narrow doorways and step-over showers, and homes with steep entries on sloping blocks. What works in one property will not work in another, which is exactly why the assessment happens on site.

Support at Home funding

Occupational therapy sits within the clinical care category of the Support at Home program, which replaced Home Care Packages from 1 November 2025. Clinical care is fully funded by the government regardless of income or assets, there is no means-tested contribution for occupational therapy.

AT-HM Scheme

Assistive Technology and Home Modifications (AT-HM) Scheme

AT-HM funding sits separate from, and is provided upfront of, the rest of a person's Support at Home budget. It exists specifically to pay for the equipment and modifications that keep people safely at home, and it is regularly left unused because people do not know it is there.

Funding is provided across three tiers within a 12-month access period:

Low tier

up to $500

Medium tier

up to $2,000

High tier

up to $15,000, with the possibility of more where the evidence supports it

Complex equipment and any home modification generally require assessment and prescription by a health professional, usually an occupational therapist.

Cognitive Change

Living Well with Dementia and Cognitive Change

Cognitive change affects daily function long before it affects memory alone. Our Occupational Therapists assess how dementia, mild cognitive impairment, stroke or other conditions are affecting safety, routine, decision-making and daily activity, and provide environmental modifications that reduce confusion and risk, simplified routines and task set-ups, kitchen, medication and wandering safety strategies, meaningful activity planning, practical education for family and carers, and clear recommendations on the level and type of support required.

NDIS

NDIS Occupational Therapy in Sydney

We also support NDIS participants across greater Sydney. Our Occupational Therapists complete Functional Capacity Assessments, assistive technology and home modification assessments, SIL and SDA reports, housing and living arrangement reports, and provide capacity building therapy for adults with neurological, cognitive, physical and psychosocial disability.

We support self-managed, plan-managed and NDIA-managed participants, and work directly with support coordinators, plan managers and Local Area Coordinators.
