Assistive Technology & Home Modifications (AT/HM) Scheme
Our occupational therapists are experienced in the AT/HM Scheme under Support at Home. We accept referrals for initial assessments and in-home trials to determine the most appropriate assistive technology or home modifications for your client.
Initial Assessment Referral
Comprehensive OT assessment to identify AT or home modification needs. Detailed clinical report aligned to AT/HM Scheme requirements.
In-Home Trials
Facilitate in-home trials of assistive technology to ensure the right equipment is prescribed prior to purchase.
Clinical Reporting
Clear, detailed clinical reports to support approval and funding decisions by the care manager.
Make an AT/HM Referral
To refer a client for an initial AT/HM assessment or in-home trial, contact our team. We'll confirm eligibility, arrange the assessment, and provide a clinical report within agreed timeframes.
